As a service to you, we will submit a claim to your insurance company for all visit charges, but we do not share the contract between you and your insurance company. Please remember that your health insurance is a contract between you and your insurance company. It is your responsibility to know your health plan benefits, including co-payment, deductibles, co-insurance, and lab contractual amounts.
You are responsible for any charges not covered by your insurance plan. We want to inform you that some services may fall under unlisted claims, which are not covered by insurance. As a result, you may be responsible for paying out of pocket for these services.
*There will be separate fees for the surgery, anesthesia, and the facility.
If you have a deductible that has not been met, The Foot Clinic will collect payment towards your deductible prior to your surgical appointment. We will contact you prior to your appointment after we have completed the Prior Authorization process.
Refunds for over-payment may be made to you only after the claim has been adjudicated by your insurance. It is not uncommon for providers to appeal the insurance decisions on denied claims. This process can take from 2 months to 1 year to complete. In general, it can take up to 4 weeks for the insurance company to process and re-process each
claim.
